YOUTH LEADER CHECKLIST
Listed below is a checklist detailing all of the forms and information needed for participation in a TeenServe mission trip. Please try to have all forms completed by May 1st.
- All Workcampers are registered
- All Adult and Teen Workcamper Forms are completed
- All Adults and Teens have completed a Release Of All Claims Form
- All Vehicles are entered into the system
- All Adults have had Background Checks
- Certificate of Liability has been sent to TeenServe
- Deposits and Payments are made
